Yeah, the cover is cute but from what I can tell the story involves Suzaku seeing some porn and asking Lelouch if he can be like a girl in a porn video and give Lelouch a blowjob. Lelouch agrees and decides Suzaku looks really hot covered in semen and so then screws. It ends with them in bed together.
